Graduate Passport Requirements

WSU requires proof of COVID vaccination for students system-wide with limited exemptions for medical or religious reasons. If an exemption is approved, the student can participate in educational activities within WSU so long as they follow all health and safety requirements (e.g., masking, social distancing, etc.) subject to their accommodation. However, even with an approved exemption/accommodation from WSU, students may still need to obtain approval from a clinical training site to engage in a clinical experience at that site per the Governor’s and Department of Health Mandate.  Clinical training sites may deny an unvaccinated student’s request for an exemption, which may impact their progression in the program.  Clinical training sites that have approved exemptions previously may discontinue exemptions in the future at their own will.

Licensure

Advanced Practice License

  • Requirement: Only required for students returning to grad school who currently have an advanced practice degree.
  • Documentation: Enter date of renewal/expiration and upload copy of licensure from Department of Health website.

WA State or Out-of-State RN Licensure

  • Requirement: Per WAC 246-840-522, all MN/DNP students must hold a current, unencumbered RN license in each state or United States territory where practice or clinical experiences occur. You must provide proof of your current RN license. You can obtain proof of WA licensure at Provider Credential Search.
  • Documentation: Enter date of renewal and upload copy of licensure from website.

Background Checks

Background Check GSA (RCW 43.43)

  • Requirement: A GSA screen is required the first year and is included in the National background check. Annually thereafter OIG/GSA checks will have a $18.00 fee which includes monthly checks. Additional GSA checks may be required for placement at certain clinical sites.
  • Documentation: Enter date of first check (done with National Background check) and date of any additional background checks thereafter, attaching a screenshot for each check.

Background Check-National Certification (RCW 43.43)

Background Check Instructions:

  • Go to: Castlebranch
  • Click on the + by Please Select to expand the window to the list of campus options
  • Select the Campus where you are admitted
  • Select the program you are in – Graduates
  • Then select the correct option for you – for Spokane this would be “WM96bg: Click here to get your background check”.
  • Read the info on the next page that pops up.
  • Click on the “I have read this information” box.
  • WSP, OIG and GSA screens are also included in the national background check.

Documentation: Enter date of check and upload documentation of official results.

  • Requirement: This is included in the National background check done upon admission. WSP results should be separated out and uploaded to WSP requirement field. WSP Watch is required annually thereafter.

Required Forms

Criminal History Disclosure Form

  • Requirement: Students much complete and upload the Criminal History Disclosure Form annually.
  • Documentation: Enter date of disclosure form completion and upload completed form annually.

Liability Insurance Form

  • Requirement: Students are required to purchase liability insurance annually with no lapse in coverage. Students can elect to purchase liability insurance offered by WSU, the application form can be found online. If students elect not to purchase liability through WSU, they must provide proof that they are covered by another carrier as a student (this may be an additional role for your carrier so check with them) at the coverage caps required by WSU (1,000,000 per occurrence & 3,000,000 aggregate).
  • Documentation: Upload of copy of liability insurance application form showing payment has been made. For personal insurance, upload a copy of the liability policy summary/face sheet showing coverage amount and dates of coverage/expiration or liability insurance application form indicating payment has been made.

Student Authorization to Release Records Form

  • Requirement: Signed student release of information, which allows WSU to send student records to clinical agencies to facilitate clinical placement and onboarding
  • Documentation: Upload signed release of information into E*Value.

Student Media Release

  • Requirement: Completed media release form. Students are not required to give their permission, but are required to complete the form indicating their preference.
  • Documentation: Upload  completed form to E*Value.

Training

CITI Basic Human Subjects

  • Requirement: CITI Basic Human Subjects Training – Social and Behavioral Research or Biomedical Research is completed upon admission by all PhD and DNP students. CITI Basic Human Subjects Training Instructions
  • Documentation: Enter date of completion and upload completion certificate.

CITI Responsible Conduct of Research and Conflict of Interest

Training – BLS/CPR

BLS Provider card from an American Heart Association (AHA) course. A course from another organization that states they meet AHA guidelines does not meet requirement. Must be renewed every two years with no lapse in certification.

Documentation: Enter date of completion and upload American Heart Association card.


Immunizations/TB Testing

All immunizations require entry of vaccine dates and upload of official documentation from healthcare provider or immunization registry showing proof of vaccination and/or titer as required by each vaccine category. Certificate of immunization forms (tri fold cards) completed by parents for K-12 school enrollment do not meet requirements.

  • Hep B – Completion of Hep B vaccination series and a positive titer at least 6-8 weeks after completion of series is recommended. If Hep B titer is negative, student can get a challenge dose (#4) followed by another titer 6-8 weeks later. For the titer, proof of Hepatitis B surface antibody (Hep B surface antibody or HBsAb) test result should be provided (Not hepatitis B surface antigen (HBsAg) which tests for active infection. Positive titer alone or with challenge dose record is acceptable if full Hep B vaccination records cannot be located. Hep B declination may be signed if you choose, but signing a declination may exclude you from placement at some clinical sites.
  • Influenza – Vaccine required by October 1 annually. If you choose not to get the vaccine, an Influenza Waiver Form must be signed annually, but signing a declination may exclude you from placement at some clinical sites.
  • MMR -Completion of 2 MMR vaccine series at appropriate interval OR positive titer to all three components of MMR (measles, mumps and rubella).
  • TB Status
    • Requirement: Documentation of an initial 2 Step TST is required and documentation of annual 1 Step TST’s (without lapse) since 2 Step was completed. Alternatively, a negative IGRA (T-Spot or QuantiFERON blood test) is acceptable in lieu of a 2 Step TST.For a 2 Step TST, the student first receives an initial PPD injection, followed by a reading 48-72 hours after the injection. Between 7-21 days after the first skin test injection, the student receives a second PPD injection, followed by a reading 48-72 hours after second injection. Documentation must show the dates of both injections and both test readings, indicating the correct interval of 7-21 days between the two TST’s.Annually thereafter, a 1 Step Tuberculin Skin Test or IGRA (QuantiFERON TB Gold or T-Spot) is required if it has been less than 12 months since last test.If it has been more than 12 months since last TST or IGRA test, a 2 Step TST (Tuberculin Skin Test) or IGRA (QuantiFERON TB Gold or T-Spot) is required.
    • Documentation: Enter date of testing for TST Or QuantiFERON and upload results from testing facility.
    • If newly positive TST or TB IGRA results, you must follow up with healthcare provider (chest X-ray, symptoms check and possible treatment documentation of absence of active M. TB disease) and need to complete health questionnaire TB Symptom Check Form.
    • If history of positive TST, please provide documentation of initial positive TST reading, provide proof of chest X-ray documenting absence of M. TB, medical treatment and negative symptom check OR TB Symptom Check Form. If history of BCG vaccine, TB IGRA. If negative, OK; If positive, do Chest X-Ray. In addition, all students with a history of a positive PPD will need to complete the health questionnaire TB Symptom Check Form annually.
  • TDap – One time immunization with Td booster every 10 years
  • Varicella – Completion of 2 vaccine series at appropriate interval OR positive titer

Other Important Information About Passport Requirements

Documents containing records for more than one passport item only need to be uploaded once. For other passport items that are found within that document, simply list in the notes field of the item where the evidence of completion can be found. For example, a document stored in the Hep B #1 field might also provide evidence of TB testing, MMR titer, etc. In the notes fields of those items, state that documentation for that item can be found in the Hep B #1 row.

IMPORTANT: Allowing a passport item to expire could result in a registration hold, a delay or cessation of your rotation until the item is updated and documented, not being able to count clinical hours accrued during the time when a passport item is expired, and/or Performance Improvement Plan).

Please be aware that some clinical sites may have additional requirements above and beyond the clinical passport. Students will need to complete these additional requirements in order to do a rotation at these sites.

*Washington State Law RCW 43.43 requires that a disclosure statement must be made by applicants to clinical/internships sites concerning certain civil adjudications, conviction records of crimes against persons, and disciplinary board final decisions. Be apprised that you may not be eligible for licensure in the state of Washington if you have certain criminal convictions or other adjudications on your record. Further, because of requirements for clinical placements and internships, if you have certain criminal convictions or other adjudications on your record, you may not be able to complete your degree, regardless of whether you are admitted to the program.

**WSU College of Nursing will make every effort to support clinical placement in states where students are already licensed (Oregon or Idaho only) but note that due to possible limitations of faculty licensure and/or availability of clinical site visitors, we may not be able to consistently place students in their home state for clinical experiences. Additionally, MN/DNP students may find having a Washington State RN license beneficial throughout their program (i.e. more clinical placement options and more timely placement).
