You will care for patients, delivering care with an emphasis on compassionate service to improve the quality of life. You’ll work autonomously and in collaboration with all members of our healthcare team to coordinate quality and consistent primary care services.
Position Details
Location: Adult family homes, memory care and assisted living communities(within 25 miles of your home)
Manage complex patients, focused on reducing hospitalizations and improved health outcomes.
Monday- Friday schedule with an ability to schedule patient appointments into condensed workdays as desired.

Requirements
Passion for delivering compassionate care to patients with chronic illnesses
Current unrestricted NP/PA-C license to practice in the state for which you apply.
Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
Intermediate computer skills and proficiency required
Home Office, High Speed Internet and Reliable Transportation Required.
Preferred Qualifications
2+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP) or Physician’s Assistant (PA-C), with experience in mobile primary care within Geriatric care, Internal Medicine, Psychiatry, and Podiatry.
Experience in managing chronic diseases (CHF, hypertension, diabetes etc.)

Pay:
$150,000.00 – $200,000.00 per year
