Our patients reside in various communities, including assisted and independent living, memory care, and adult family homes. We also provide in-home care for patients with unique healthcare needs. Our team is led by board certified Family Practice and Internal Medicine Physicians and patient care is directly driven by Nurse Practitioners and Physician Assistants specializing in Geriatrics and Long-term Care. Duties:
- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
- Diagnose and treat acute and chronic illnesses, injuries, and infections
- Prescribe medications and develop treatment plans
- Provide health coaching and education to patients and their families
- Collaborate with physicians and other healthcare professionals to coordinate patient care
- Perform procedures within the scope of practice
- Utilize evidence-based practice guidelines to deliver high-quality care
- Maintain accurate and up-to-date medical records
Qualifications:
- Valid Nurse Practitioner license in the state of Washington
- Certification in the respective specialty area
- New Grads welcome
- Excellent clinical skills and ability to provide compassionate patient care
- Strong communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team

Job Type: Full-time
Pay: $130,000.00 – $160,000.00 per year
